FHWBC Assessment Questionnaire

This Assessment Questionnaire is useful for church leaders to check the Biblical balance of their congregation. The Questionnaire should be distributed first as this Assessment Questionnaire is used to score the results. It is used in conjunction with the Foundations of a (Biblically) Healthy Well-Balanced Church Course Presentation. The questionnaire should normally be given to all congregants and leaders as well. After this, the Assessment is used to score and diagnose the results.

(It is advised that leadership answers be kept separate in order to keep from skewing the congregational assessment. It also allows the leadership to see where there might be substantial differences between the leaders and the congregation on matters of spiritual importance.)

There is a separate answer sheet for the questionnaire that should be used in scoring the questionnaires to determine where there are strengths and potential weaknesses needed correction.
